VODI-IMPACT: A Healthy Living Campus is a collaborative health delivery model providing hypertension and diabetes prevention and management services to underserved residents in a high-risk, northeast Detroit community. The IMPACT campus – the primary hub for services – is where IMPACT members have ready-access to health, medication and nutrition education, health care, fresh foods, and fitness activities, all in a safe and supportive environment. The goal of VODI-IMPACT is to reduce the incidence, prevalence, and severity of diabetes and hypertension in the Conner Creek and Osborn neighborhoods through healthy lifestyle choices.
The IMPACT Health Living Campus is one of eight national Safety Net Enhancement Initiative (SNEI) sites funded by a grant from the Kresge Foundation with the goals of:
- Exploring the role of community health centers (CHC’s) in population health
- Understanding how cross-sector partnerships impact health disparities
- Developing models to inform the field, and
- Building a framework for “community health systems”
SNEI grantees are charged with making a measurable impact on the health of vulnerable communities by addressing the social determinants of health care and community engagement, including neighborhood safety, access to health resources, health education and culture.
